Request Form

Welcome to the PARED My Maintenance system. To log and track maintenance requests for PARED schools, please complete the form below:

  1. Complete ALL mandatory fields.
  2. Use the room number as the location to help identify and track issues.
  3. Enter only ONE issue per request.
  4. Select your site in the "WORKS DEPARTMENT" field (e.g., MON = Montgrove, Tan = Tangara, etc.)
  5. Submit a photo of the issue. This allows the team to respond quickly to your request.

The relevant works department will be notified upon submission. All requests are logged, prioritised, and assigned within My Maintenance. You will be informed of the status via your work email.
